Terms and Conditions of Adoption
The staff of the Fort Smith Animal Society (FSAS) is very interested in finding the most suitable homes for our adoptable pets. In accepting this animal you must agree to be bound by the following rules and conditions.
I understand that the FSAS reserves the right to refuse adoption of any animal for any reason to any person. Reasons for rejection/refusal of adoption applications are not required to be given.
Adoption to out of town college students and those in college housing is prohibited.
There will be no animals adopted to anyone under the age of 18 years.
There will be no animals adopted out to anyone appearing to be under the influence of alcohol or drugs
Adoptions will be refused during the Christmas holiday season.
I understand that the FSAS cannot guarantee the health, temperament or training of the animal herein applied for and will not be responsible for the nature or the condition of the adopted animal after it leaves the Society’s care.
I realize that no animal will be adopted out for any purpose other than as a household pet.
I agree to provide the animal with a good home, proper care, and kindly treatment throughout its life.
I assume all risks of injury to, from, or loss of my pet.
I am aware that the society will not release an animal to persons under 18 years of age.
I agree to arrange with veterinarian/technician for all necessary immunization vaccinations, and provide all further preventative and medical care in the future.
I agree to have the animal neutered/spayed within sixty (60) days or in the case of young animals, prior to six (6) months of age and to present proof of spay/neutering to the FSAS. (If spay/neuter is done by Stickney Veterinary Services in Fort Smith ˝ the cost may be reimbursed by the FSAS upon presentation of receipt showing the sterilization has been carried out.) The animal being adopted will be surrendered to the FSAS on demand if proof of spay/neutering is not presented to the FSAS within the time frame specified.
Prior to release of an adopted animal all Town of Fort Smith licensing requirements must be met.
All pets require training or housebreaking to prevent behavioural problems and permit adjustments to family, and/or other pets. I accept full responsibility to provide proper training.
I realize that under sections 444, 445, and 446 of the Criminal Code of Canada, it is an offence to abuse an animal. A person found guilty of animal cruelty could be subject to a maximum $2,000.00 fine and/or six (6) months in jail under the Criminal Code of Canada.
It is required that all animals be implanted with Avid Micro-chips for permanent identification at the time of adoption placement and the animal registered to the owner, this is done at no additional cost
Adoption fees are not refundable under any circumstances.
